Secure Upload Portal
Please use this form portal to safely and securely upload any benefit, new hire, banking, etc. documents you need to submit to the Employee Engagement Office:聽
*Click on the PDF聽fillable聽form link you wish to submit to the secure portal for processing, select聽诲辞飞苍濒辞补诲听辞谤听save as聽(right click your mouse) and save this form to a folder on your computer.聽 Once the form is saved/downloaded on your computer, close the form.聽 Open the form you saved on your computer, complete the form and save it again.
Once you upload your form via the portal, please email the聽hr@daemen.edu聽to let us know the form has been submitted.聽
